A foreign investor may plan to buy specialized vehicles in Ukraine, contract with private clinics, rehabilitation centers or hospices, and transport patients between facilities, home after treatment, or to a medical institution. At first glance, this can look like a transport business. The legal model changes when medical professionals accompany the patient, monitor the patient’s condition, use medical equipment or provide medical assistance during the trip.
Before purchasing a vehicle or hiring a team, define what the service will actually do. The practical launch sequence is: define the service model, establish the Ukrainian operating structure, determine whether a medical practice license is required, build the team, prepare the vehicle and equipment, resolve special-signal issues, prepare operating documents, and then enter into regular contracts with healthcare providers.
Define the Patient Transportation Model Before Applying for a License
Transportation Without Medical Care
Not every transfer of a sick, elderly or mobility-impaired person is automatically medical practice. Ukraine’s Classification of Economic Activities (KVED) draws an important distinction. Class 86.90 includes ambulance transportation services provided in critical situations, but excludes transportation of patients without life-support equipment and without medical personnel, referring such activity to transport sections instead.
A business that only moves a passenger from one location to another, without a medical team, medical monitoring or care, may therefore have a primarily transport-based legal model. The exact transport requirements require separate analysis, but the presence of a patient alone does not automatically create a medical-practice licensing obligation.
Medical Transportation with Patient Monitoring or Care
The position changes when the service includes healthcare activity during transportation. If medical staff accompany the patient, monitor vital signs, use medical equipment or provide medical care, the project should be analyzed as a healthcare activity before launch.
Ukrainian law defines medical practice as licensed healthcare business activity carried out to provide medical or rehabilitation assistance, medical services and medical care. For an investor, the practical question is not what the vehicle is called, but what the team will actually do inside it.
This distinction should be fixed before capital is committed. A vehicle selected for non-medical transport may be unsuitable for a licensed medical model, while an “ambulance” concept may create staffing and regulatory requirements not included in the original budget.
Choose the Ukrainian Business Structure
A foreign investor needs a Ukrainian operating structure through which the service will employ staff, contract with clinics, use vehicles and, where required, hold the medical practice license.
For a project with several vehicles, employees, B2B contracts and plans to scale, a Ukrainian limited liability company owned by the foreign investor is usually a practical starting model. This differs from a small owner-operated model where a Ukrainian sole proprietor may be appropriate.
The ownership or use of each vehicle should also match the operating structure. If a car belongs to another group company, a founder or another third party, the operator needs a documented legal basis for using it.

Select the Correct Activity Codes
For medical patient transportation, KVED 86.90, Other human health activities, is the key classification reference because its official description includes ambulance transportation services, including by aircraft, provided in critical situations. The same class expressly excludes transportation of patients without life-support equipment and medical personnel.
The code should follow the actual operating model. The presence of 86.90 in registration data does not by itself create or replace a medical license. Other healthcare codes, such as 86.21 for general medical practice or 86.10 for hospital activities, should be added only if the business actually performs those activities.
Activity codes should follow the actual medical and operating model rather than being added simply because the business transports patients.
Obtain a Medical Practice License
Where the transportation model includes medical care or medical services during the trip, the operator should be prepared for the Ukrainian medical-practice licensing regime. The license should be planned before the vehicle is purchased, not treated as a formality afterward.
Ukraine’s Licensing Conditions for medical practice are currently in force in the version effective from September 1, 2026. Earlier amendments adopted in July 2025 entered into force on January 8, 2026, and the 2026 changes further updated staffing categories, qualification rules and the list of specialties.
Before filing, align three elements:
-
Who will be the licensee. For an international investment project, this will often be the Ukrainian LLC operating the service.
-
Which medical specialties and healthcare professionals are required by the services actually provided during transportation.
-
Which equipment and material resources are needed for that model.
The licensing documents should describe one coherent operating model: the declared services, specialties, staff and equipment should correspond to each other.

Build the Medical Team for Patient Transportation
The team should be designed around the medical tasks performed during the trip. There is no safe universal formula under which every patient-transport vehicle must always have the same combination of doctor, paramedic, nurse and driver.
If the service provides emergency medical assistance, emergency-medicine qualifications may be central to the model. If it provides another form of monitoring or care, the required specialties may differ. The staffing model should therefore be matched to the scope of care before recruitment.
Under the current Licensing Conditions, the business must have at least one healthcare worker, including a part-time employee, for each declared specialty. Qualification requirements are tied to applicable professional standards or, where such standards do not exist, to qualification requirements approved by the Ministry of Health.
Foreign healthcare professionals create an additional layer of work. Before relying on a foreign doctor or other medical professional, verify whether the person’s education and professional qualifications are recognized in Ukraine and whether employment formalities are required.

Prepare the Vehicle and Medical Equipment
The vehicle is part of the operating model, not merely a transport asset. Its configuration depends on the patients, the medical team and the care that may be provided en route.
At the planning stage, assess:
-
patient transport and immobilization equipment;
-
monitoring equipment;
-
oxygen support;
-
emergency or resuscitation equipment where required;
-
emergency medical supplies.
Stretchers, transfer chairs, pulse oximeters, blood-pressure monitors, glucometers, manual resuscitation bags and oxygen equipment may be relevant examples, but they are not a universal checklist. The final equipment list should be determined after the medical specialties, staff model and scope of care are defined.
Before buying or importing a specialized vehicle, also check whether its technical configuration, registration status and intended use are compatible with the planned service and any applicable certification or registration requirements.
Special Lights and Sirens Are a Separate Legal Issue
A medical practice license does not automatically give a private patient-transport vehicle the right to use blue lights or a siren.
Ukraine has a separate Ministry of Internal Affairs procedure for permits to install and use blue special light and sound signaling devices. The current rules apply only to defined categories of vehicles, including emergency medical care teams. A private operator therefore needs to establish whether the vehicle and service actually fall within an eligible category.
A patient-transport company does not obtain emergency-vehicle privileges merely because it employs medical staff or holds a medical practice license. Eligibility depends on the vehicle’s legal status and actual function.
Where the vehicle qualifies, the permit process also requires compliance with the applicable color scheme, identification markings, inscriptions and signal-device requirements. The current MIA procedure refers to DSTU 3849:2018.
Check this issue before branding the fleet or installing special signals. Otherwise, the investor may spend money on a configuration that cannot legally be operated as planned.
Prepare the Service for Day-to-Day Operations
Obtaining a license is only one part of the launch. The business must also be ready to operate the vehicle, document transfers and manage incidents consistently.
Vehicle Documentation
The operator should have documents confirming ownership or lawful use of the vehicle, registration documents and any additional documents required for the particular operating model. Where the vehicle is leased or provided by another entity, the contractual and registration arrangements should be checked before it enters service.
Technical and Equipment Control
Verify whether mandatory technical control applies to the specific vehicle and how often it must be completed. Medical devices should be reviewed separately: metrological verification is not automatically required for every item, and the applicable control depends on the specific measuring instrument and regulatory rules.
Medical Records
The medical record set should follow the actual type of care. Forms used by emergency medical teams should not be treated as universally mandatory for every private transport operator without checking their applicability. Determine what must be recorded before, during and after the transfer, including the patient’s condition and handover.
Internal Protocols
Internal procedures should address sudden deterioration in the patient’s condition, escalation to emergency assistance, communication with the receiving facility, patient handover, incident documentation and other critical events. The aim is to prevent the team from improvising legal and medical procedures during a difficult transfer.
Contracting with Private Clinics and Other Healthcare Providers
A common B2B model is a service agreement between the transport provider and a private clinic, rehabilitation center, hospice or other healthcare provider.
The contract should reflect the actual service. At minimum, define:
-
the scope of transportation services;
-
whether medical care is included;
-
who transfers and accepts responsibility for the patient;
-
how trips are ordered and confirmed;
-
the parties’ responsibilities;
-
which documents accompany the patient;
-
payment terms;
-
what happens if the patient’s condition changes.
A clinic is not automatically prohibited from contracting with a provider that does not hold a medical practice license. The decisive issue is what the contractor actually does. Non-medical transportation and transportation that includes medical care are different legal models.
A Practical Launch Sequence for a Foreign Investor
- Define whether the service is medical transportation or non-medical patient transport.
- Choose and register the Ukrainian operating entity.
- Determine the medical specialties, staffing model and activity codes.
- Select or purchase the vehicle only after the service model is clear.
- Prepare the vehicle, equipment and personnel for medical licensing where required.
- Separately assess eligibility for special lights and sirens.
- Prepare operating documents and contracts with healthcare providers before launch.
This order reduces the risk of buying an “ambulance” first and then trying to fit the legal, staffing and licensing model around the vehicle.
Legal Support for Launching a Medical Transportation Service in Ukraine
Legal Service Ukraine can support the project from the initial business-model review through operational launch. We can determine whether the planned service is medical or non-medical transportation, structure the Ukrainian operating company, identify the licensing scope and staffing requirements, and review the planned vehicle and equipment before major investment is made.
Where a medical license is required, we can prepare the licensing documentation, align the declared specialties with personnel and material resources, and support the application process. We can also assess the special-signal issue separately rather than treating it as an automatic consequence of medical licensing.
For the operating stage, our lawyers can prepare B2B agreements with clinics and other healthcare providers and develop internal documents for the chosen patient-transport model.
